A $200 non-refundable deposit is due at the time of application for Full Day Kindergarten. For priority consideration, payment must be received in the school office by Monday, February 13, 2023. Please make checks payable to OLMC. Bring or mail to school office 14596 Oak Ridge Rd. Carmel, IN 46032. We are not able to accept online payments.
What documents do I need to apply?
Copy of birth certificate
Copy of baptismal certificate (and 1st Communion certificate if applying for grades 3-8)
Copy of current immunizations
Copy of most recent report card & standardized test scores (Grades 3-8)
Have current teacher (if applicable) complete & mail the Teacher Questionnaire available on school website:
Your application is not considered complete until all of the documents listed are received by the school office including the completed Teacher Questionnaire from your child’s current teacher.
